I understand that by completing this application there is no guarantee of a job interview or a job offer. No promises or guarantees regarding employment have been made to me by National Job Agency Ltd. Statements or personnel guidelines or in my communications with any National Job Agency Ltd representative is intended to create an employment contract between National Job Agency Ltd. and me. Additionally, I understand that if an employment relationship is established I have the right to terminate my employment at any time and I also understand that National Job Agency Ltd retains the right to terminate my employment at any time. Further, I understand that National Job Agency Ltd. has the right to modify its policies without giving me any notice at the change(s).

I hereby authorize National Job Agency Ltd. to verify all the information I have provided on my application. I also agree to "execute as a condition of employment or continued employment any additional written authorizations necessary for National Job Agency Ltd. to obtain access to copies of records pertaining to this information. I expressly authorize National Job Agency Ltd to contact any of my prior employers from any liability whatsoever (except liability under the civil rights laws) arising out any of my information requested or disclosure.

I certified that I can and will, upon request, substantiate all statements made by me on this application, that all such statement, dishonest answer, misrepresentation, removal of omission to any question will be sufficient for rejection of my application, removal of my name from the eligible list or my immediate discharges should such falsifications or misrepresentations be discovered after I am employed.

Once Temporary or Permanent placement has been made, National Job Agency Ltd. requires the following:

1) That you are legally entitle to work in Canada.

2) That you are ready, willing and able to start working on specified date on time and you remain reasonably available throughout the contract period.

3) That you understand that National Job Agency Ltd. reserves the right to terminate your placement at any time, should the client be less than satisfactory with your on-site performance or shortage of work.

5) That the employee is restricted from soliciting work from clients or National Job Agency Ltd. for a 1 year period.

6) That the employee must give National Job Agency Ltd. a 24 hours notice prior to your schedule shift if you are unavailable, otherwise it's understood that you may be removed from that placement and put on a "will call" list.
